Missing the default android menu on Atrix 2

I'm using Motorola Atrix 2, Android 4.0.4. After few app ugrade, i have noticed the android default menu is not there in all downloaded menu. But the is still install, just the icons of them has missing in menu drawer. i have do the factory reset.

When i want to do the factory reset from setting menu..the menu is not there. I google the way to do the factory from the hardware of the phone. You can do it if your phone get stucked or hanged.

Here is the way to do factory reset from the hardware of the phone:

1. Remove the battery and wait for a few minutes
2. Reinsert the battery and turn on your phone
3. Turn off your phone
4. Press Power Button and Volume UP & DOWN Keys until the boot mode menu show up with blue colour font 
5. Press the Volume DOWN Key to hidhlight the Recovery option
6. Then press Volume UP Key to select and start RECOVERY
7. Your phone will reboot, then show the Android Robot
8. When the Android Robot appear on the screen, press Volume UP & DOWN together.
9. The Recovery menu with yellow colour font should appear on the screen
10. Press the Volume Keys to scroll to the Factory Reset option
11. Press the Power Button to activate the Factory Reset
12. Use the Volume Keys to scroll to Yes option
13. Then press the Power Button to start the Factory Reset
14. When the the reset complete, press the Volume Keys to select the Reboot System Now
